Emergency admissions of Walsall residents to hospitals continue to rise year on year and yet at the same time, many patients who are considered fit enough to be discharged are left waiting in hospital beds before their needs can be assessed. Evidence has shown that care delivered at home can enhance better health outcomes for patients.
To support this approach the Intermediate Care project recognises the need to provide a single, rapid assessment service for patients leaving hospital care, to ensure the support they receive at home is appropriate to their needs. This could include physio, home-help, specialist services and equipment that promotes independent living for people following post-acute care.
By adopting this model of care, we will ease the pressure on A&E admissions by freeing up beds for those who really need them, and help people live more independently at home instead of admitted into a care home environment.
